HOME VISITS

At Health in Abundance, we are committed to providing accessible and compassionate care to all our patients. Home visits are available under specific circumstances to ensure the safety and wellbeing of those who are unable to attend the surgery in person.

Home visits are offered exclusively to current registered patients who are experiencing extenuating circumstances that prevent them from attending the practice. These visits are provided at the discretion of the patient’s General Practitioner (GP), based on clinical need and appropriateness.

Requesting a Home Visit:

  • Patients or their family members/carers should contact the practice directly to request a home visit.

  • Requests should be made as early in the day as possible to allow adequate time for clinical assessment and scheduling.

  • If necessary, the practice team may ask for relevant clinical information to help the GP assess the urgency and necessity of the visit.

Assessment and Booking:

  • The GP will review the information provided and determine whether a home visit is clinically appropriate.

  • If approved, the practice will advise the patient or their representative of the expected time frame for the visit.

  • In some cases, alternative arrangements may be recommended if a home visit is not deemed necessary.

AFTERHOURS CARE

Health in Abundance does not provide direct after-hours medical care.

If you need urgent medical assistance outside our regular business hours, please contact GP Access After Hours on 02 4926 0500.

GP Access is designed to support patients whose health concerns cannot wait until regular business hours but do not require emergency department care. Their service operates on a triage-based system. When you call, their staff will assess your condition over the phone and provide advice. If needed, they will arrange a prompt appointment at one of their five clinics.

If you’re unsure whether your issue is urgent:

For any medical emergency, call 000 immediately or go to your nearest emergency department. Examples of emergencies include:

  • Chest pain

  • Difficulty breathing

  • Facial or limb weakness

  • Facial swelling

  • Extensive burns

  • Severe injury

  • Sudden loss of or change in consciousness

URGENT & WALK-IN APPOINTMENTS

Health in Abundance FMP operates on a pre-booked appointment basis and is not a walk-in clinic.
 
To maintain high standards of care and ensure timely service, a limited number of same-day appointments are reserved for unforeseen urgent medical concerns. These are assessed by our reception and clinical staff and allocated based on clinical urgency and availability.

Walk-in patients are not given priority over those who contact the practice by phone. All requests — whether in person or by phone — are triaged equally according to clinical need.

If you have an urgent medical issue, please call the practice at 9:00 AM on the day. This allows us to manage your care promptly and efficiently.

Once all urgent same-day appointments are filled, patients with pressing medical needs will be placed on our daily triage list. This list is actively monitored and discussed with our GPs throughout the day to ensure timely attention to urgent matters. A GP also reviews the list before it is finalised at the end of the day.
